SHIPPING INSTRUCTIONS
Step 1: Complete RX Form – simply use the RX form provided to place a single order. Just send the 1st copy (white copy)
Step 2: Prepare your case. Include impression, models, bite registration and RX Form into an individual plastic bag and secure with a tape or rubber band.
Step 3: Pack your case. Place the case materials into a box. Place as many cases as possible in the box in order to save shipping cost. For better protection, place foam with bagged cases.
Step 4: After you sealed the box, you can drop it at the nearest UPS drop box or hand it to the UPS driver. Please call us on (609) 882 7876, if you need prepaid ground shipping labels.
TURNAROUND TIME
7-10 business working days for crown and bridges and all kinds of removable prosthetic (days are counted upon receipt of the case).
